To sponsor a memorial tree, print this page, complete the form below, enclose a check payable to "City of Evanston - Ladd Arboretum Account", and mail it to the City of Evanston, Parks Division, 2100 Ridge Avenue, Evanston IL 60201, attention Paul D'Agostino. Gifts are deductible for Federal Income Tax purposes. Please contact the Parks Division at (847) 866-2912 if you have any questions.

Existing Trees
Dedications for existing memorial trees that have not been dedicated are based on the following scale:

New Memorial Trees
It is also possible to add tree varieties not currently available in the arboretum. Note that there is limited space available for new memorial trees. Donations for new memorial trees are based on the following scale:

• Botanical name of the tree (i.e. Acer rubrum)
• Common name of the tree (i.e. Red Maple)
• Name of person to whom the tree is dedicated

Please indicate how you want the name to appear on the plate. A maximum of 21 spaces are available. This includes periods and spaces between first and last names. Please print the name on the line below exactly the way you would like it to appear on the name plate:
